Comment: 3. You are subject to the following special conditions:a) You shall complete one hundred (100) hours of community service at thedirection of your supervising officer and submit verification and proof ofcompletion to your probation officer. This community service shall beconcurrent with any other community service you are ordered to complete.b) You must successfully complete any training or counseling program yourprobation officer tells you to take and you are solely responsible for provingthat you are attending the programs your probation officer has directed youto take.c) You must also take and successfully complete Outpatient Substance AbuseTreatment at the level recommended in a current substance abuseevaluation, and provide proof of attendance to your probation officer. Youmust also participate in and successfully complete Cognitive Self Change,MRT, or other thinking errors class as approved by probation officer andprovide proof of attendance to your probation officer. You must alsocomplete Parenting class approved by probation officer. You must alsocomplete Seeking Safety class approved by your probation officer.d) You must make an appointment with a community health care providerwithin 14 days of sentencing to complete a psychiatric medication evaluation, management, and education; and then enroll in and successfullycomplete individual and/or group therapy; and case management asrecommended in a current mental health evaluation.You must attend 3 self-help sober support meetings per week for the first 6months, in a group chosen by the Defendant and including obtaining asponsor. After that for the period of time specified by the probation officer.You are not to have contact with Co-Defendant Jacob Lyson unless approvedby IDOC probation officers after a period of time where both havedemonstrated compliance with probation terms.You must provide a DNA sample and thumbprint impression, as required bylaw.
